To request promotional items:

1. Download and save the Request Form to your local drive. Please note that filling out the form in your browser won’t enable you to submit it so you must save it to your local drive first.
2. Fill out the form. Please be sure to include your name, the customer name and number (plus all pertinent shipping information if we are shipping to the customer).
3. Click on the “Submit Form” button. This will open an email in your email application and attach the form to it. Then click “send” and it will be sent to North America Business Unit Marketing so we can process your order.
